This year for our Back to School Bash (B2SB) at Life Church we are having STUDENT TAKEOVER on Sunday, August 11! On this special day, students in MOTION (grades 1-5) and SWITCH (grades 6-12) will be TAKING OVER the services at Life Church. If you are a parent filling out this form - please let us know the name of your child and where he/she would like to serve. They will have a choice between the choir or the hospitality team. We will also need their t-shirt size. Please select the student's choice and t-shirt size from the menu below.

*Please note: this is only for students in Motion and Switch. Additionally, if you choose the choir, your student will need to be available on Sunday, August 11 from 7:15am - 12:30pm (this includes the pre-service run through and all 3 services).
